Peripheral Osteoma at the Inferior Border of the Posterior Mandible in a Young Female: A Case Report

Peripheral osteomas of the mandible are rare benign neoplasms, comprising less than 1% of jaw tumors and typically affecting adults aged 30-50 years, with exceptional occurrences in younger patients, warranting syndromic evaluation such as Gardner syndrome. A 25-year-old asymptomatic female presented with a slowly enlarging, painless, hard mass beneath her mandible, noticed over 12 months, without systemic history or trauma. Extraoral examination revealed a firm, immobile 2-cm lesion at the inferior border of the posterior mandibular buccal site, broadly attached to cortical bone, with normal regional lymph nodes and no intraoral findings. Panoramic radiography showed a well-defined sclerotic radiopaque mass near the mandibular angle. Provisional diagnoses included peripheral osteoma, osteoblastoma, and peripheral ossifying fibroma. Under general anesthesia, the lesion was excised en bloc with periosteal margins and submitted for histopathology after 10-day decalcification in 20% nitric acid. Hematoxylin-eosin staining demonstrated mature lamellar compact bone with Haversian systems and no atypia, confirming peripheral osteoma. Postoperative recovery was uneventful, with no recurrence at 6-month follow-up and restored function. This case underscores the diagnostic challenges of mandibular peripheral osteomas in young adults, emphasizing histopathological confirmation and surgical excision as curative, alongside screening for syndromic associations to guide management and enrich the sparse literature on non-syndromic variants.
