Periapical Actinomycotic Infection: Review of published Case Reports

Background: The purpose of this study was to gather current information on the epidemiology and treatment of actinomycotic periapical lesions by summarizing prior research on clinical and management strategies for prevention and treatment.
Methods: In this review, recent case reports available in full were selected from the PubMed and Google Scholar databases, focusing on publications between 2015 and 2025 that specifically addressed Periapical Actinomycosis.
Conclusion: This study indicates that practitioners should remain vigilant for peri-radicular actinomycotic infections, particularly in instances of recurrent sinus tracts and inadequate responses to conventional treatments. Confirmation of this diagnosis should be achieved through biopsy and microscopic evaluation. The proposed management strategies for periapical lesions associated with actinomycotic infection varied among patients, depending on the extent of the lesion and the risk of developing a cervicofacial infection, including endodontic retreatment, periapical surgery, and/or antibiotics, or 0.2% chlorhexidine mouthwash, with prolonged follow-up.
